Consent is given to participate in the activity/program listed. In consideration of the agreement of the Case-Barlow Farm Board to provide the activity/program to my child and/or to myself, I release the Board, its officers, trustees, agents, and volunteers, acting on behalf of the Board, from any and all liability claims or demands for personal injury, sickness, or death, as well as property damage expenses of any nature whatsoever which may be incurred by the undersigned and/or the child participant that occurs while my child or I are participating in the activity.
I assume on behalf of myself and my child all risk of personal injury, sickness, death, damage and expenses as a result of my or my child's participation in the activity/program. I further agree to hold harmless and indemnify the Board, its officers, trustees, agents, and volunteers, acting on behalf of the Board, for any liability sustained by the Board as a result of any negligent, willful, or intentional acts of the child or by myself. I consent to the use of my or my child's photograph on the Case-Barlow Farm website or in publicity of the Case-Barlow Farm.
I understand that due to the nature of Case-Barlow Farm's kitchen and the size of the event, Case-Barlow Farm is unable to guarantee an allergen-free meal. Guests with severe or life-threatening allergies should take appropriate precautions if choosing to participate.
I understand the event will promptly start at the communicated time and date. Late arrivals will miss any food or activities that occurred prior to their arrival.
